Registration Form-Please fill out the registration form completely. If there
are areas of the form that do not apply to you put N/A in the space provided.
Make sure you sign and date giving the Jaguars consent for your child to be
treated by another physician and to play for the Jaguars team.
Physical Form-Schedule your child’s physical early. Each player must have
a physical on file with the Jaguars before he can start practice.
Player’s/Parents Code of Ethics-Make sure you read this with your child.
You and your child will need to sign the form.
Fees-The fee for your child to participate is $140.00. $100.00 goes toward
Helmet, shoulder pads, 7 piece set, game jersey, game pants, mouth piece,
game socks, banquet and trophy, and participation in one Bowl Game. The
remaining $40.00 goes toward the $960.00 per team city registration fee.
Payment-You may pay the $140.00 up front and participate in a fundraiser
where you can sell 50 World’s Finest Chocolates at $2.00 per bar with
$50.00 going toward your son’s football jersey or you can pay the total of
$190.00 if you don’t want to participate in the fundraiser. Jaguars will accept
payments starting July 28th – August 8th.
Practice-Practices will be held at Gammon Elementary, 3240 N. Rushwood,
from 6:30pm-8:00pm starting Monday, July 28, 2008.
*We will need a copy of a birth certificate or PIF form on
file. You can get a copy of the PIF from your child’s school.
